Incident Reports

Quick reference for when and how to file an Incident Report in the kennel system.

Purpose

An Incident Report is the kennel system record of any unusual event that happens with a dog in our care. Use this quick reference for when a report is required and what to put in it.

At a Glance

  • File one for any unusual event with a dog in our care.
  • Notify the Daycare Manager.
  • Fill it out immediately, with as much detail as possible.
  • Not logged into your own account? Write down your name as the staff member filing the report.
  • Beyond-a-band-aid employee injury on company property is mandatory, no exceptions.

Immediate Actions

  • Find the Incident Report form in the kennel system.
  • Document the unusual event as an Incident Report and notify the Daycare Manager.
  • Fill it out immediately, not later in the shift, with as much detail as you have.
  • Include: the time the incident occurred, any staff involved, any injuries, and the actions taken afterward.
  • If you're not logged into your own kennel system account, write your own name in the report as the staff member who filed it.
  • Any employee injury on company property that needs more than a band-aid gets a report, every time. This keeps a record of anything that could turn into a bigger issue for everyone involved.

Who to Ask

The Daycare Manager is who gets notified when you file a report. For the fuller response and reporting chain once a fight or a bite is involved, see Managing Fights and Bites and Managing Emergencies in Daycare.

Frequently Asked Questions

What should new hires know before deciding whether an Incident Report is required?

Any unusual event with a dog in our care gets one. For an employee, the threshold is an injury that needs more than a band-aid while on company property. When in doubt, file it and notify the Daycare Manager.

What should new hires know before filling one out under someone else's login?

Write your own name in the report as the staff member who's filing it, even though you're not logged into your own kennel system account. The record still needs to show who documented the incident.

What should new hires know before deciding how much detail to include?

As much as possible, filled out immediately rather than from memory later: the time it happened, which staff were involved, any injuries, and what actions were taken afterward.
